If you don't want to pay the $99 developers fee but want to test your app on a device there are ways to patch Xcode and your project. However these are frequently complicated and difficult to follow. I have just found http://jailcoder.com/ and it's a very simple guide and you can test on your device in under a minute, it's that easy.
I can speak from experience using Lion 10.7.4 and the latest Xcode which I believe is 4.5.2 -> testing on iPod 3rd Gen 5.1.1.
