You forgot about ioreg using Chameleon.

Clover DSDT "Fixes" should be avoided. Especially when you already have a patched DSDT.

Check system.log for USB related errors.

WOW, You're the best RehabMan. I'm sorry I forgot to attach ioreg of chameleon. But I already fixed it. You said "Clover DSDT 'Fixes' should be avoided", so I unchecked all Fixes and you know it fixed the problem. Now all ports are working as usual.
