Yes I used a few ones between v5071 from sourceforge and the current v5112.

Yes it's here already.
I have never run into this before on any of my builds. From your descriptions of the way the UEFI/BIOS settings are affecting the system I would say it is something in the BIOS implementation and not in the OS's or in Clover.
 
Well... after reading this message from another thread I though "what if the culprit was Above4G decoding?". Disabled it, disabled CSM and now BOOM I can properly boot in both macOS and Windows with both iGPU and dGPU fully enabled, and get proper resolution during boot.

However I don't know if it is more a kind of a UEFI bug or if it is related to my very dGPU/iGPU combination or maybe both. Anyway, I fixed my issue.
